NVB1 Form

Photo ID (e.g. Passport or drivers licence)

Evidence of current address dated within the last 3 months (e.g. Bank statement or Utility bill)

Proof of ID for under 18's. One of the following is acceptable: Birth Certificate or Passport or a Written statement by a principal officer confirming attendance at educational institution on a letter head of that institution

If you are aged 16-18 years, you will need to submitted signed “Parent /” consent form
